Objective of CGHS

  • To provide comprehensive medical care facilities to the central government employees and members of their family
  • To do away with the cumbersome and expensive system of reimbursement of medical expenses

CGHS CARD

  • CGHS identity card is issued to all CGHS beneficiaries.
  • Index card one each in office and dispensary
  • It contains a unique card number
  • Colour coded – Green, Blue and Pink
  • It also contains name of cardholder’s department, residential address, name & dates of birth of all family members and group photo of all family members
  • Card is issued by the government servant’s department for serving employees
  • Card is issued by the Additional director CGHS for all others
  • CGHS contribution is deducted from government servant’s salary every month
  • Temporary authority is issued to beneficiaries visiting other cities having CGHS facility
  • CGHS card should be kept safely
  • It should be produced at every visit
  • In case of loss
  • Inform police, CMO I/C of dispensary
  • Duplicate card will be issued on production of police FIR and application
  • Pensioners to apply to the Additional Director for duplicate card

PLASTIC CARDS

  1. Individuals may apply online for issue of plastic cards through the Link provided on the Ministry of Health’s website < www.mohfw.nic.in>.
  2. They will have to have individual photographs  in electronic/ digital form for uploading. The same maybe obtained from any good photo studio or existing images may be scanned and uploaded where required.
  3. These Plastic cards are valid during the duration of Service of an individual and have to be renewed every periodically with fresh photographs.
  4. Superannuated individuals shall be issued such plastic cards too in due course. The Photographs in this case shall be scanned for uploading when applying for cards.
