PERMISSION FOR PURCHASE OF CPAD FLUIDS FROM OFFICE OF THE ADDITIONAL DIRECTOR

 

Documents required:

1.      Request letter addressed to the Additional Director, Bengaluru for purchase of CPAD fluids.

2.      Advise of Government nephrologist recommending CPAD and indicating the consumables (CPAD fluids and Y SET, Transfer set, Mini cap etc) required, along with the number(amount) of each item required every month.

3.      Other relevant medical documents of beneficiary illness.

The permission is accorded for a period of six months.

Reimbursement procedure:

a.      Covering letter from Card holder.

b.      Duly filled claim form and bank mandate form and other relevant documents.

c.       Copy of CGHS card.

d.      Bill/Receipt (original) indicating the details.

e.      Paid proof.

f.        The original or photocopy permission letter.

g.       Utilisation certificate from the treating physician.

 

In case of serving employees, the permission shall be obtained from their parent department.
