PROCEDURE FOR OBTAINING HEARING AID FROM OFFICE OF THE ADDITIONAL DIRECTOR

 

Documents required to be submitted by pensioner beneficiary at parent Wellness Centre:

1.      Request letter from Card holder.

2.      Original letter of Recommendation of ENT specialist indicating left/right/both ear and the type of hearing aid.

3.      Audiogram report duly authenticated by ENT Specialist.

4.      Undertaking from Card holder that he has not been reimbursed the cost of hearing aid in the preceding five years for the said dependent.

5.      Copy of CGHS card.

Other terms:

       Repair and maintenance of the hearing aid is the responsibility of the purchaser.

       The permission is given for a hearing aid carrying warranty of at least three years.

       The permission to other city card holders can be given. However, reimbursement shall be preferably from their parent Wellness Centre/ CGHS city.

       Replacement may be allowed after five years.

Reimbursement procedure:

a.      Covering letter from Card holder.

b.      Duly filled claim form and bank mandate form and other relevant documents.

c.       Recommendation of ENT specialist indicating left/right/both ear and the type of hearing aid.

d.      Audiogram report duly authenticated by ENT specialist.

e.      Undertaking from Card holder that he has not been reimbursed the cost of hearing aid in the preceding five years for the said dependent.

f.        Copy of CGHS card.

g.       Bill/Receipt (original) indicating the details of the hearing-aid seller ie. name, qualification, and RCI/MCI registration number.

h.      Paid proof.

i.        The original permission letters.

j.        Empty box/boxes or the cartons with the label showing the details of the hearing aid.

k.       Copy of Warranty card for three years.

The reimbursement is restricted to prevailing ceiling rates fixed by CGHS or the actual whichever lower
